Breaking a Lease?

Is there a form letter someplace of what to say when you notify your landlord you are breaking your lease? I may be relocating, and if I do, I will have to break the lease on my apartment.

My landlord has not provided me a copy of my lease. After five months and several letters, nothing has happend. A certified letter requesting my copy of the lease was refused. So I am not sure if the lease indicates that 30 days notice is required or anything.

Also, I have one more concern... if my landlord habitually refuses certified letters, how can I be sure that I am providing notification to him that I will be breaking the lease?

Any suggestions or ideas are welcome.
