Your Par was returned to you specifically because DHCR did not/would not sign for the certified letter. You send all your paperwork again, and include copies (not the originals) of the Certified mailings, ie the green card, the certified mailing receipt, and the envelope it was mailed in. Attach a cover letter referencing the return of your certified mailing, the copies of proof of mailing, etc.

Send it out twice, Fed-Ex and certified mail again. My experience is that DHCR is totally disorganized, loses most everything so you must combat that by sending everything twice.
