Is this a new landlord? When my building got taken over by a new LL, he sent that form, but it wasn't a rider attached, it was just on his stationary and it was information he wrote that would be for his files.

If this is the same LL, then I'd ask about it. He should have had this information already.

The Renewal form does have a rider but that's not the rider. Call DHCR for more information.

: Attached to my Renewal Lease Form is a rider, printed on my landlords stationary, called an information sheet, asking my name, employers name, address & phone number, emergency numbers to call, and whether I have air conditioners installed, and if I have a washing machine installed.
: Do they have any right to this type of information? I have rented here for the last 20 years.
: Can I just return my completed Renewal Lease Form without this rider without any repercussions?
