Sounds like the guy who had to leave had a grudge against the primary tenant of the apartment so he decided to get in a "parting shot".

The primary tenant should respond to the inquiry with the facts. If the ex roommate pursues this in court, tenant should present her side and the judge in all likelihood will dismiss the case. The roommate could land in serious legal trouble if he tried to present forged documents in court.

In short - its annoying, but you've got to follow through, and you will win. The only way to prevent this is to screen roommates very carefully and to demand rent as soon as it is due, rather than waiting several weeks to commence legal action.
