Yes, if rent-regulated or if it is in your lease. Read whichever applies. NOTE: 'sublet' means you don't live there anymore.

Yes, for all tenants. But if the landlord asks, you must provide the roommate(s) name(s) within 30 days. (is it 30? look it up: search in this forum for 'roommate law')
