Lease renewal offer

Hi all!
The landlord offerred lease renewal form TP-8.

He sent a photo copy of the front of the form only.
Back instructions are completely missing.

Paragraph 7 says that written agreement is attached.
The agreement is missing and for the last 14 months
the landlord refused to cure.

The questions are :
1. Does it make the lease renewal ofer invalid?
2. Does it make the lease unenfrceable?
3. Does it make the his right to deemed renewal rent unenfrceable?

Please refer me to laws and regulations and/or case citations
covering the above issue.
