Eviction Notice

My name is on the lease and my roomates is not. The situation is not working out. How do I obtain a 30 day notice. Is it a standard form, a formal letter, or is it something notarized. What type of 30 day notice will stand up in court if I have to give it to some one? Are they served it in person, certified mail? If they do not abide by it then I take them to court?
